We want to hear from you and your constituents about the customer service being provided by state agencies. We believe that the more feedback we receive, the better we can design and improve our state services.

To help us collect and track this feedback, we have created a form where you can submit concerns or comments regarding state agencies.

What You Can Expect by Filling Out This Form

After you submit the form, Your Washington staff will follow up with the relevant agency to better understand the issue and determine the next steps the agency is taking to resolve it. Once our review is complete, Your Washington staff will provide you with a summary of our findings.

Your Washington staff will also remain available to assist with any follow-up requests you may have after the issue has been addressed.
